AMA ADULT Annual Release 2023 (18 AND UP)
AMA MINOR Annual Release 2023 (UNDER 18)
AMA SX Credential Terms and Conditions
Anti-Doping Rider Consent Form

Non-US residence must upload a copy of their Passport
W-9 Tax Information
Direct Deposit Enrollment Form
Copy of your Insurance Card
ImPACT Concussion Test (every two years)
Email Paul R. Reiman, M.D., Medical Director at mxdoc951@gmail.com or baseline@themedicrig.com to request Home Testing information. You will be sent an email (within 3 business days) with your own individual code for the test and the required video instructions for accessing the test. Plan for 30-45 minutes for the test. After completion of the test, be sure to upload your confirmation.
