Requirements:
Prior to Registration, you must have received and completed the Attendance Identification form from Feld Entertainment. If you have not received it or completed please e-mail healthdocs@feldinc.com to obtain it prior to registering.
Prior to approval, the following documents must be submitted/uploaded:

- AMA ADULT Annual Release 2021 (18 AND UP)
- AMA MINOR Annual Release 2021 (UNDER 18)
- AMA SX Credential Terms and Conditions
- FIM Anti-Doping Rider Consent Form
- You may also need to update your Concussion Test (every two years). Email Dr. Reiman at mxdoc951@gmail.com. In the subject line place Impact – Home Test. Invitation expires in 90-days (Cost to rider is $20). You will receive an invitation within three days. Upload a copy of your confirmation here.
DOCUMENTS THAT NEED TO BE UPLOADED (DURING THE REGISTRATION PROCESS)- ALL RIDERS
- Non-US residence must upload a copy of their Passport
- Upload copy of your insurance card
- FIM Medical Examination Form Appendix B.pdf , MUST BE SIGNED AND STAMPED BY DOCTOR (450SX ONLY)
DOCUMENTS THAT NEED TO BE UPLOADED (DURING THE REGISTRATION PROCESS)- NEW RIDERS
- Birth certificate
- Direct Deposit Form (Any purse will be direct deposited into your account by the Tuesday after the event)
- W9 - Request for Taxpayer Identification Number and Certification (FOR U.S. RIDERS WITH U.S. SOCIAL SECURITY NUMBER)
- Form 8233 (FOR FOREIGN RIDERS WITHOUT U.S. SOCIAL SECURITY NUMBER)
